Who is eligible?
All active employees scheduled to work 30 or more hours a week are eligible. Benefits are effective first of the month following 60 days of employment.
Your eligible dependents may include:
- Your legal spouse.
- Your child(ren) up to age 26 including natural children, legally adopted children, and stepchildren.
- Your child(ren) over age 26 who are not able to support themselves due to a physical or mental disability.
Please Note: Enrolling someone who is not qualified as a dependent is considered insurance fraud. If enrolling dependents under any plans, please provide HR with documents showing proof of relationship.
When can I enroll?
You can enroll only at certain times throughout the year.
- As an employee, you can enroll during the annual open enrollment period.
- If you are a newly hired employee, you must enroll or waive coverage within 30 days from your date of hire.
- Employees enrolling due to a qualifying life event change must enroll or waive coverage within 30 days from the date of the qualifying life event.
How do I make changes?
Open Enrollment is the one time each year that employees can make changes to their benefit elections without a qualifying life event.
- You must notify Human Resources promptly by submitting your qualifying life event in Employee Navigator if you need to make a change to your benefits. You will be required to provide acceptable supporting documentation for your benefit elections. Please note, you have 30 days from the date of the qualifying life event to make any changes.
- Qualified changes in status include, for example: marriage, divorce, legal separation, spouse's change of employment status, birth or adoption of a child.
