Hospital Indemnity, Critical Illness & Accident Insurance
Hospital Indemnity Insurance pays you benefits when you are confined to a hospital, whether for planned or unplanned reasons.
Critical Illness Insurance pays lump sum cash benefits directly to you in the event of critical illness. You can use this cash benefit as you wish to help pay for any out-of-pocket expenses you may have.
Accident Insurance helps you deal with unexpected expenses. Benefit payments help with medical deductibles and covers household expenses.
These benefits are administered through Prudential.

HOSPITAL INDEMNITY HIGHLIGHTS
Hospital Admission
Pays $1,000 - Up to 5 time(s) per calendar year
ICU Admission
Pays $2,000 - Up to 5 time(s) per calendar year
Hospital Confinement
Pays $200 - Up to 30 days per confinement; payable to a maximum of 5 confinements per calendar year. When an admission benefit is paid, the confinement benefit pays on day 2
ICU Confinement
Pays $400 - Up to 31 days per confinement; payable to a maximum of 5 confinements per calendar year. When an admission benefit is paid, the confinement benefit pays on day 2.

•Employee: $10,000 increments to $30,000
• Spouse: 10,000 increments to $30,000 $10,000 increments to $30,000; not to exceed 100% of Employee Amount
• Child: 5,000 increments to $15,000; not to exceed 50% of Employee Amount
