Voluntary Life and AD&D Plan Insurance

E.L. Haynes offers Voluntary Life and AD&D insurance to all eligible employees. You pay the full cost for this coverage. Employees must elect voluntary coverage in order to elect coverage for dependents. Only newly eligible employees during initial enrollment are eligible for the guarantee issue amount without having to provide evidence of insurability (EOI). If you do not elect any coverage, you are considered a late entrant and are subject to an EOI. An EOI is required for amounts over guaranteed issue or if you enroll after your initial eligibility period. An EOI is not required for child life insurance. If an EOI is required, it must be approved by the insurance company before you purchase coverage. This benefit is 100% employee paid. Please see the chart below for details.

- Coverage is only available to the spouse and dependent child(ren) if the employee elects coverage.
- Spouse’s rate is based on the employee's age.
- Guaranteed Issue is only available when employees are initially eligible to enroll in benefits: as a new hire, or as an employee who is newly eligible to enroll (i.e. switching from a role that is not benefits eligible, to a role that is benefit eligible). If you waive coverage during your initial enrollment, you are not eligible for the Guaranteed Issue amount at Open Enrollment or during a Qualifying Life Event.
- Spouse coverage terminates at age 75

Disability
We offer Short-Term Disability (STD) and Long-Term Disability (LTD). These disability coverages work in conjunction with one another to provide continuous income replacement in the event of a sudden income loss.

- STD is a company-provided benefit. You are eligible for the plan automatically at no cost to you.
- LTD is a company-provided benefit. You are eligible for the plan automatically at no cost to you.
- Coverage amount is based on weekly pre-disability earnings.
- The duration is subject to medical documentation and additional information provided by your doctor.
