Critical Illness Plan
Administered by Aflac
Employees can choose between $15,000 and $30,000 of coverage. Benefits are paid as a percentage of the amount of coverage elected. Your benefits under this plan are guaranteed issue and not subject to pre-existing condition limitations as long as the insured is in complete remission and treatment-free for 12 months. 100% coverage amount for critical illness diagnosis of cancer, vascular, and other specified illness. Rates are based on the insured’s age, tobacco status, and benefit amount. Dependent children are automatically covered for 50% of the employee’s election amount and may be covered until the end of the month in which they turn 26.

Your critical illness insurance policy includes a $50 Wellness Benefit offer, which will pay you and covered family members an annual benefit if you complete an eligible health assessment/screening.
Tips For Filing a Claim
The document(s) must show dates of hospital stay and diagnosis. It is helpful to have one or more of the following when submitting your claim for hospital admission and confinement benefits under the sickness rider:

We offer a voluntary Accident plan through Aflac to regular full-time employees and their qualifying dependents. This benefit helps fill financial gaps caused by expenses related to an injury caused by a covered accident. Cash benefits are paid directly to you, regardless of what is covered by your medical plan or any other insurance. Benefits are paid for initial and follow-up care, medical imaging, X-rays, dislocations, fractures, physical therapy, and more. These benefits can be used to pay expenses like coinsurance, deductibles, or everyday expenses like your mortgage, childcare, or groceries. It’s up to you.

We offer two voluntary Hospital Indemnity plans through Aflac to regular full-time employees and their qualifying dependents. These plans pay cash benefits to you for an inpatient hospital admission and daily stays. These benefits can help pay towards your medical plan’s deductible, coinsurance, or everyday expenses like day care, utilities, and groceries.
Please notice that only the Low Plan is compatible with a Health Savings Account. If you enroll in the High Hospital Indemnity Plan, you are not eligible to contribute to a Health Savings Account.

NEW! Guaranteed Life with Long-Term Care Benefits

Aflac’s fully portable Guaranteed Issue Life Insurance with a Long-Term Care (LTC) benefit provides employees and their families with lifelong protection and added peace of mind. Coverage is available for employees and their dependents, with employee face amounts ranging from $10,000 up to $150,000. Eligible employees can enroll during open enrollment without medical underwriting, and the premium rate locked in at the time of enrollment will remain the same and will not increase in the future.
In addition to permanent life insurance protection, this two-in-one coverage includes benefits for long-term care services that employees can access during their lifetimes to help pay for the high cost of LTC. The plan also builds cash value over time, which can be borrowed against or used to purchase a smaller paid-up policy with no additional premiums due.
Once coverage is purchased, employees can keep it as long as they want. The benefit amount remains level and does not decrease as employees age, providing valuable protection during working years and into retirement.
