Voluntary Life Highlights
Employees who would like to supplement their group life insurance may purchase additional coverage. As long as you purchase coverage for yourself, you may purchase coverage for your spouse and children as well. For this coverage, you are responsible for the full cost. Guarantee Issue is only available when you are first eligible for coverage.

Please note - Voluntary Life and AD&D is 100% employee paid
- Coverage is only available to the spouse and dependent child(ren) if the employee elects coverage.
- Spouse’s rate is based on the employee’s age.
- The Guarantee Issue is the dollar amount that will be issued without submitting proof of insurability, such as answering medical questions on an application or completing a medical exam. Check with your employer to see if you’re eligible for the Guarantee Issue.
