To-dos:
- Review all benefit options within this guide.
- Login to PlanSource to elect benefits, using Google Chrome or Firefox.
- Select Current Benefits (Open Enrollment) or New Hire Enrollment (New Employees) to begin selections.
- The Enrollment Confirmation page lists all elected benefits. You can download this page for your records.
- Choose Checkout at the bottom of the screen.
- Username: First initial of your first name + First six letters of your last name + Last four of your SSN (i.e. JSmith1234)
- Password: Your date of birth in the YYYYMMDD format (i.e. 05/02/1977 = 19770502)

During benefits Open Enrollment, you have the opportunity to review your coverage needs, consider the benefits plans available to you, and select those that will provide the most value to you and your family.

Benefit News - Life & Disability Carrier Change
Effective April 1, 2026, our Life and Disability benefits will transition from The Standard to New York Life.
What's Changing
The following plans will move to New York Life:
- Basic Life & AD&D
- Voluntary Life
- Short-Term Disability (STD)
- Long-Term Disability (LTD)
What You Need to Know
- Your current coverage will automatically carry over — no action is required.
- A special open enrollment period will be offered for employees who want to make changes.
Special Open Enrollment: March 2 - March 13, 2026
During this time, you may:
- Enroll in or increase Voluntary Life coverage.
- Enroll in Short-Term Disability (STD).
These options are available with Guaranteed Issue, meaning no medical questions are required for:
- Increasing Employee Voluntary Life up to $300,000. (Amounts above $300,000 require medical questions)
- Increasing Spouse Voluntary Life by up to $25,000.
- Enrolling in Short-Term Disability.

Who is Eligible?
Full-time employees working 30 or more continuous hours per week are eligible for the benefits outlined in this overview.
- Your spouse (legally valid existing marriage)
- Your dependent children until the age of 26 (employee's natural, newborn, adopted, foster, step child(ren), or a child for whom Employee has been court-appointed as legal guardian or legal custodian)
- Your grandchildren until 18 months (newborn child of a covered dependent child)
How do I Enroll for Annual Enrollment?
- Annual Enrollment is October 1 - 31, 2025.
- All employees who wish to make changes to their current benefits MUST complete online enrollment.
- The deadline to complete online enrollment is Friday, 10/31/25.
When Does Coverage Begin?
- Annual Enrollment - Benefits begin on 1/1/2026.
- New Employees - Benefits begin on the first of the month following 30 days of employment.
Changing Your Benefits
You may make changes to your benefits if you experience a qualified life event.
- Examples of QLE: marriage, divorce, birth, adoption or a gain or loss of coverage by your spouse or dependent child.
- Your change must be consistent with the Qualified Life Event which has occurred.

Qualifying Life Events
Dependent Documentation
Qualified life event requests and supporting documentation must be submitted in PlanSource within 30 days of the date of your life event. Supporting documentation must contain the reason for the change, the date of the event, and the family members who are affected by the event.
IMPORTANT: If you do not request a change in the enrollment site or do not provide the documentation within 30 days, you will have to wait until the next open enrollment to add or drop yourself or your dependents.
Changes to your elections are governed by the Section 125 Plan.
Mid-Year Changes / Life Event Documentation
You may make changes to your benefits elections if you experience a qualified life event.
All changes must be submitted within 30 days and must be the result of and consistent with the qualified life event that occurred.
