VOLUNTARY LIFE AND ACCIDENTAL DEATH & DISMEMBERMENT (AD&D) BENEFIT HIGHLIGHTS
If you elected voluntary life coverage in the past, your benefit will continue into the next plan year. If you wish to make any modifications to your current elections, you may do so. For employees, you can purchase or increase your current coverage by $20,000 without completing a statement of health. For spouses, they can purchase or increase coverage by $10,000 and coverage on children can be purchased or increased by $5,000 (maximum benefit is $10,000).
If you wish to elect an amount over those amounts, please complete a Statement of Health form and your amount request will be reviewed. Please see Employee Navigator for rates and necessary enrollment forms.

1. Coverage is only available to the spouse and dependent child(ren) if the employee elects coverage.
2. Spouse’s rate is based on the spouse's age.
3. The Guarantee Issue is the dollar amount that will be issued without submitting proof of insurability, such as answering medical questions on an application or completing a medical exam. Check with your employer to see if you’re eligible for the Guarantee Issue.
