This is your one-time opportunity to elect up to the Guarantee Issue amount without having to complete an Evidence of Insurability form and answer medical questions. If you wish to elect over the Guarantee Issue amount, then an Evidence of Insurability form is required. Please contact Human Resources for the Evidence of Insurability form and please see the Rippling portal for the rates.

1. Coverage is only available to the spouse and dependent child(ren) if the employee elects coverage.

2. Spouse’s rate is based on the employee’s age.

3. The Guarantee Issue is the dollar amount that will be issued without submitting proof of insurability, such as answering medical questions on an application or completing a medical exam. Check with your employer to see if you’re eligible for the Guarantee Issue.

4. From Birth to 14 days: Max coverage $1,000
